  • When booking for a group, please use each person's individual name, phone number, and email so they can fill out their intake forms. If this is a surprise then use your phone number.
  • We send intake form links via text or email. Forms must be completed before your appointment - unfinished forms will need to be done on-site.
  • For clients under 18, a parent/guardian signature on our waiver is required.
